Alert when a task or workflow fails

Is there a way to alert, preferably email when a task and/or workflow fails?

Currently not specifically to a task. However if the task failure causes a provisioning of an instance to fail it would email a user with notifications on that their instance failed to provision.

Thanks! We have custom post provisioning workflow and tasks. It would be great feature to have I think. Have a checkbox, email upon failure or successful with email address input field.
