i am getting this issue intermediately , Same catalog is provisioning instance successfully but sometimes its failing
Unfortunately that is not enough logging to be able to fully tell what is going on. Typically, that is along the lines of a template failing to clone. (Image not found, etc).
This may be better handled via a support portal ticket.
